    My first thought when I saw the title was, oh another unlucky person lost a wheel on the track. Geez, how can something like this happen?? I don't even think he heard of a wrench, haha. He or his crew seems to only hand tighten the lugs. Its first time I've seem some loose all four wheels.

    The theory is that they put on 1/2" nuts on 12mm studs, and used an impact wrench.

    That's crazy. All the damage done to the car too. Lucky he was auto-crossing it instead of running high speed on a track with other cars.

    I bet you can't do that even if you planned it !!

    how did that happen ? even if it were just hand tight, I can imagine all 4 coming off at same time.

    They were the wrong sized lug nuts as thefrush said. The nuts were only grabbing the very outside lip of the thread, so once any stress was put on them, they came right off. You could have even torqued them, but they would have come loose with even a light tap of hammer.
